Submitting a Change Request

Submitting a Change Request can be done within the AHN Master. Find the AHN in question, then click on the checkbox to the left. Once the AHN is selected, click “Actions” then “Change Request.”

The next screen will ask you to select the change reason from the dropdown.

Based on the change reason selected, this will allow you to edit the fields in question. 

For example, a name change correction will only allow you to edit the AHN name. All other fields will be greyed out, and will not allow edits.

Please note that if a customer has moved locations, please use the Customer or Facility Moved change reason.

DA 

Duplicate AHN 

Duplicate AHNs will be deactivated and the earliest assigned AHN will be retained. The active AHN will show in the refer-back field in AHN history.  

MC 

Market Segment Correction 

The AHN will be retained with the corrected market segment.  

NC 

Name Correction 

The AHN will be retained with corrected name. 

TC 

Telephone Correction 

The AHN will be retained with corrected telephone number.  

ME 

Mobile Phone or Email Correction 

The AHN will be retained with corrected mobile phone number or email address 

AC 

Address Correction 

The AHN will be retained with the corrected/updated address. Allows user to make changes to address 1, address 2, city, state, zip code or country. Changing PO Box to Street Address is not allowed; request a new AHN.   

VF 

Customer Moved 

The AHN will be deactivated and a new AHN assigned. The new AHN will be shown in the refer-back field of the deactivated AHN. 

FV 

Customer Closed 

The AHN will be deactivated. Note: a facility is not closed when it changes ownership or name. Select Ownership Change (OC) or Name Correction (NC) for appropriate correction.  

OC 

Ownership Change: Clinic/Retail/Producer 

The AHN will be deactivated and a new AHN created reflecting the new ownership.
